For Member of Academic Partners (MOU)


On the mutual consent of Academic Memorandum of Understanding (MOU), Fujita Health University accepts students/Trainees/Researchers from Academic partner Universities and institutions. They must get approval and apply through their home institution.

(Overall Procedure)
1. Contact the Center for International Relations Office through your home institution.
2. After getting the acceptance from Fujita Health University, please submit the required documents below:

For Students

① Application form for International Students
Which contains information such as:
- General Details (personal information)
- Elective Choices (schedule, elective department, duration)
- Visa Requirement
- Your Identification (Copy of your student ID)
② CV (Free format)
Deadline ① and ②: At least 3 months prior to arrival at Fujita Health University
③ Itinerary Form for International Students
Which contains information such as:
-Flight Information
-Medical Insurance (including Travel Insurance)
④ Mandatory Immunization Health History Form
Deadline ③ and ④: At least 1 month prior to your arrival at Fujita Health University

For Trainees/Researchers

① Application form for International Trainees/Researchers
Which contains information such as:
- General Details (personal information)
- Visa requirement
- Your Idenfication ( copy of your student ID or university ID card)

② CV (free format)
③ Certification for Diploma
④ Certification for Medical license*
(*only for doctors)
Deadline ①, ②, ③and ④: At least 3 months prior to arrival at Fujita Health University

⑤ Mandatory Immunization Health History Form
⑥ Certificate Verification
⑦ Itinerary Form for Trainees/Researchers
- Flight Information
- Medical Insurance (including Travel Insurance)
⑧ Pledge
Deadline ⑤, ⑥, ⑦and⑧: At least 1 month prior to your arrival at Fujita Health University
